Sign Out Sheet
If you need to leave class for some reason, please ask Mr. Sinusas and he will have you sign out. Please neatly write your first and last name, where you are going, the date and time you leave. Don’t forget to write the time when you get back to the classroom. Any item that is highlighted is missing, incorrect or illegible. Make sure you are neat, thorough and accurate. When all boxes are complete, put the paper in the back of the pile. If there are no more blank papers, ask Mr. Sinusas.
If you need to go to the bathroom, try to think of the best time that will be least disruptive to the class. Leaving the classroom can be distracting to people trying to work. Even better, think of the best times during the day to use the bathroom (between classes, during lunch, tutorial, etc.) If your team teachers notice a habit of going every period (without a medical reason), you may be put on a bathroom plan.
If you leave class during the period, don't think you are alone and no one is watching. Just outside the science room, there is a camera directly overhead. Next time you are in the main office, look at the monitor, it shows all the cameras throughout the school.
